CCM (Chronic Care Management)

Description

Ongoing care coordination and support for eligible patients with multiple chronic conditions, helping manage medications, treatment plans, and overall health between office visits. WHAT IS CCM? * An extensive care service that contributes to better patient health and care for patients with chronic conditions. * A Patient-centred care plan for all health issues, focusing primarily on management of chronic conditions. Chronic conditions are medical conditions that will last for two or more years. * The CCM program will focus on continuous patient relationships with care team members. The patient must have at least two chronic conditions to qualify. * *The CCM program is covered and encouraged by most insurances! Some exceptions apply, verify with your policy.* WHY CCM? * CCM will provide an additional resource for the patient, supporting the patient with any chronic diseases and achieving health goals. * CCM will assess the patient's medical, functional, and psychosocial needs. * CCM will oversee the patient's medication self-management and ensure that the patient receives timely recommended preventative services. * CM services are extensive and provide a structured record of patient health information, comprehensive care plan, and continuous access to care.
