Copays & Deductibles
Copays and deductibles will be collected as required by insurance companies and GA state law.

Keep your Insurance Active
Is your responsibility to maintain the status of your insurance policy up to date.

Keep your Information Updated
Ensure your billing information is correct, your insurance may deny or reject claims due to incorrect/ missing demographic information.

Unpaid Fees
Unpaid fees for services rendered are your
responsibility to monitor.

Insurance Coverage
If your insurance coverage is denied, you will be charged as self pay for the visit.

Established Patient - Selfpay $100
No show fee $45

Prior Authorizations $25

Incorrect Billing Demographics $15
Prior Authorizations (PA)
Prior Authorizations are not included in our medical services and may be required by certain insurance providers for specific medications.
To initiate a prior authorization request, patients must contact our nursing staff and indicate the medication requiring approval.
Processing time may take 3 to 5 business days.
A $25 fee will apply per prior authorization.
